Extracurricular Activities (40 Points)

Extracurricular Activities are defined as school-related activities that you are part of in addition to your regular classroom work. This includes school-sponsored clubs, organizations, associations, programs, and athletic teams. Please list your extracurricular activities as they relate to the program’s four areas of focus.

Write the full name of any activity that has an acronym (i.e. write Future Business Leaders of America, not just FBLA).

Entrepreneurial Activities (Outside of School) (15 Points)

Entrepreneurial activities can come in many forms, and you do not necessarily have to be selling a product to be considered an entrepreneur. It could be something as simple as solving a problem that exist in your community. For this section please list activities (outside of school) that can be considered entrepreneurial. It could be a business you've started, a community problem you've helped solve, or other activities that have benefited something or someone as a direct result from your involvement.

Essay Topic (35 Points)

If you received funding to start up any entrepreneurial company you wanted but it must be located in The Center's 45 county service territory, what kind of company would you start and why? Essay Requirements: 1-2 pages double spaced; .5 inch margins; and Times New Roman 12 point font. More than 2 pages will result in a deduction of points.

Academic Performance (10 Points)

You must submit a cumulative (freshman and year to date sophomore year) school transcript along with your program application. Your cumulative grade point average (un-weighted by your school) will be utilized to assess a score on academic performance. Applicants should have a minimum cumulative grade point average of 3.0 on a 4-point scale.

Letter of Recommendation

Please submit one (1) letter of recommendation with your application.
  • -Family members or relatives may not write letters of recommendation.
  • -Although this assignment is not worth any points, you will be deducted 5 points if you do not submit a letter. The letters of recommendation are used as a character assessment for the scoring committee.

Final Assignment
